Registration Form, Release Form, and Fees

To register, complete the registration form and release form, then return both forms (preferably by scanning and emailing) along with two separate checks (the old-fashioned way) for the first two fees noted below to the Instructor of Record ("IOR") as noted on the registration form.

If you have trouble downloading the forms, just leave a comment below with your email address (which will remained unpublished) to receive the forms via email.

The course fees comprise two different components, payable separately, so as to minimize the amount of money going through the course instructors:

1. Include a check payable to "NSP- Eastern Division": either $7.50 for National Ski Patrol members or $15 for others.
2. Include a separate $60 check payable to the IOR (as noted on the registration form) to cover instructor costs.
